Leave of Absence (LOA) prescriptions play a critical role in supporting patients’ recovery and employers’ visibility into workforce health. But before this project, Dialogue had no reliable way to track LOAs consistently across Canada, leading to inaccurate client reporting and inconsistent patient follow-up. I led UX strategy and design to standardize documentation, automate event logging, and improve continuity of care.


problem

Practitioners prescribed 80–100+ LOAs per week, but the workflow relied heavily on manual, inconsistent documentation:

  • LOA details were typed into free-text PDF attachments

  • “Next Steps” reminders were often missed or skipped

  • CTMs had to interpret every consult note to know an LOA existed

  • Follow-ups only happened if someone remembered to schedule them

This created client reporting gaps, operational confusion, and real patient care risk. If LOAs weren’t recognized in time, patients weren’t always contacted when their leave ended — turning care into a reactive instead of proactive system.

The core issue: CTMs were acting as interpreters of clinical intent instead of supporting care directly.

To eliminate ambiguity, I redesigned how practitioners document medical attestations by embedding a structured inline form directly into the consultation note. This mirrored an existing prescription interaction pattern — reducing learning curve and ensuring consistency.

When a practitioner indicated an LOA, the system automatically:

  • Logged an LOA event in the patient record

  • Posted structured details to the care team

  • Generated the required PDF document

  • Sent it directly to the patient

  • Flagged a follow-up task for CTMs only when needed

This approach removed 90% of the workflow burden from CTMs and ensured clinical events were recorded exactly when they happened — at the source.
